The Mission of the Regional Continuity of Care Council, RCCC, is to provide a forum for health care professionals to network, share information and develop resources. With this information, participants will have enhanced ability to provide high quality services to clients as they move through the health care continuum. Pertinent legislation and health care trends are also discussed to provide participants with the insight needed to function in the ever changing health care industry.
The Vision of the RCCC is to develop a supportive network for health care providers. Through the RCCC, trusting relationships are created and nurtured. Participants in the RCCC include representatives from hospitals, home health care, skilled nursing facilities, residential care facilities, durable medical equipment companies, governmental agencies, social service organizations and others. Together, they share information and discuss concerns from advocacy to ethical standards. Thus, the community benefits from their understanding of available resources and the timeliness of education and discussions.
The RCCC's values include promotion of high quality integrated continuity of care for our clients and the community we serve. Education, leadership and consumer advocacy are supported as avenues to the RCCC goals. Members are encouraged to maintain the highest ethical standards and to support a cooperative environment to better serve the community.
